Description: Life Skills - Online Form for Personal Information Practice completing a generic form for personal information needed for: Registering for school Registering for events Ordering food Online shopping Taking a survey Etc I made the boom card deck after watching my students struggle to complete a simple online application. For example, even though they could remember their email address they had difficulty typing it out and including the @ sign and .com. Some of them could handwrite their information correctly but need to practice typing it in formats that don't allow speech to text. This is a simple way for students to practice entering personal information in an online form including Name Address Email Address Phone number Birthday Gender Race Emergency Contacts Each slide has written instructions and examples, an audio recording reading the instructions. Click the microphone to play the recording. There are no correct/incorrect answers as the fill-in-the-blank sections are open-ended. The personal information that your student enter is not recorded or saved. The Gender and Race sections have checkboxes that include options for "other" and "prefer not to say". These sections are included because they are so common on personal information forms. It is a personal choice to disclose this information or not but this is a way to introduce the concepts of race and gender for when they are encountered on a form in real life. The last card is for extra practice completing a form.
