Description: This get-to-know-you questionnaire is a great way for older students to tell you a little bit about themselves in their own words. In my own personal speech sessions, I had students fill out a similar questionnaire and then share their answers (if they wanted to) with their group, so they could learn about things that they had in common with their peers. It's a great ice breaker for new configurations of groups, and for SLPs new to the caseload. This questionnaire was designed for upper-grade students to fill out independently, and reviewed by the SLP. You can assign this questionnaire to students if they are part of your classroom on Boom Learning. If you assign a fast-pin, you will not be able to review their answers.
